Job Description
The Pain & Palliative Lead monitors trends in pain management and promotes consistency in documentation practices and also oversees the implementation and evaluation of the palliative care program, ensuring practices align with Extendicare’s person-centered care approach.
Core Accountabilities
-Monitors Clinical UDA Dashboard
-Monitors PCC Clinical Documentation
-Connect with home leadership as required and provide updates related to: program trends and follow-up required by other interdisciplinary team members
-Attends monthly Restorative Care Coordinating meetings participates and collaborates with the interdisciplinary team
-Tracks QI scores through PCC insights or equivalent
-Participates in Continuous Quality Improvement quarterly meetings and provide the Quality Improvement Committee with guidance and recommendations.
-Participates in Staff Training and Education
-Supports home leadership in completing the annual program review
-Perform full program audit on required schedule
