Job Description

Daily Responsibilities

Daily aspects of the ADOC role would include, but are not limited to:

Review of staffing levels for assigned units and ensuring plans in place to mitigate impact of any shortages and high-risk issues. Attendance to dining room/s on assigned floor to observe meal service (throughout the day or as needed). Review of the 24-hour report/72-hour report (after holidays, weekends. LOAs). Flagging the following risk issues for further discussion with Registered Staff to ensure risks are mitigated and immediate action taken on serious/high risk issues, that the plan of care is up to date/in place and all required assessments and documentation are also up to date: 

  • Falls
  • Transfers to hospital
  • Behaviours
  • Skin and wound issues
  • Infections (Review New Infection Symptoms Report)–daily
  • Pain Management
  • Risk Management
  • Hydration issues
  • Issues warranting CIS reports.
  • (Suggested process: review the report in PCC and make notes as per above. Ensure you have a system for follow up to ensure completion such as check off through the day when items are addressed or if continued follow through required to carry forward to next day to do list.  Suggest keeping notes are high risk issues on a day-to-day basis for subsequent referral back to as required.)
  • Review dashboard views for risk issues, over-due care plans,
  • Throughout the day, review PCC progress notes, UDAs for high-risk residents, discuss with PSWs and Registered staff as to their assessment, feedback, and plan to address.
  • Ensure POAs are notified of any incidents or follow up required.

Daily rounds on assigned unit/s to touch base with RN/staff. Discuss if he/she is aware of what is in report – discuss f/u, what is due for today (weights, restraints, infections, falls, skin, pain. A question to ask them is “which resident/s are they most concerned about and what is the plan to address this concern?”

  • ensuring care is provided as required (as per care plan),
  • residents are well groomed, sensory and communication aids in place.
  • unit if free of odours
  • PPE signs and supplies are available and infection practices are adhered to (hand hygiene for residents, staff, and visitors)
  •  all hazardous substances are stored as required, personal items are labeled
  • Oxygen signage is in place
  • Privacy and confidentiality is maintained
  • Equipment is being checked as required and risks are reported as well as equipment is clean, disinfected as required (including resident ambulatory equipment)
  • Call bells in reach, falls management interventions in place (falling star, mats, lower bed)
  • Meal service begins on time; registered staff are in attendance as required
  • Staff have name tags on
  • Interact with residents, families, staff
  • Restraints in use on walk through are reflected on tracking sheet

Complete Staff Huddles on each unit -> provide updates, education and collect pertinent information. Daily – ask about updating of care plan, random verification of care plans of residents with acute issues to ensure reflective of current care requirements. New Admission – checklist. Review daily to ensure all aspects up to date. When checklist is completed, ensure Reg staff sign off and document is given to the ADOC Notify and update DOC and Admin of any high-risk issues

Weekly Responsibilities

Attend Care conferences if required –; IDTC for admission and high-risk families/residents at minimum. BSO meetings with BSO team to discuss current case load, issues Attend all RAPS – Schedule to be determined. Attending – 20 mins/each. Ensure process for RAPs meetings is followed as detailed on map Attend CMI meetings Provide Education & Support to staff ongoing or as gaps are identified.

Monthly Responsibilities

Nursing Practice Meetings Ensure Committee Meetings occur as per schedule. Ensure Program Evaluations are being completed as per schedule Ensure minutes from meetings are completed in a timely manner following meeting and copies are in their respective binders. Review of QIP, MOH action plans to ensure all commitments are up to date, timelines are adhered to. Review of education in Surgelearning to ensure mandatory education is completed as per schedule (schedule to be developed) Run PCC reports as and when required (MDS V2 QI, clinical summary, assessment scoring, weights, UDAs completed as required (eg pain, skin and wound, falls), CMI/RUGs for your units. Work with LTC consultant to ensure gaps in Nursing Standards Review (Home Status Reports) are addressed Run MDS V2 QI report or PCC Insights to see what residents on your units triggered. Follow through as required (e.g. restraints – are consents in place, is documentation correct).

  • Medication Management
    • Med Pass

Review PCC for missing meds (3x a week) Meal Audits -> attend meal service, ensure it starts on time, all hands-on deck, processes are followed.  Monthly narcotic count sheets audit -> ensure sign off as required. Audit medication rooms for compliance to policies Debrief with Medical Pharmacy on audit and reports and ensure follow through completed. Audit Vaccine fridge temperatures
